Venous aneurysms are extremely rare, despite this, they have been reported in the majority of venous territories. The most common location for them to present in the lower extremities is in the popliteal veins, followed by the common femoral veins. It is believed that a lot of these aneurysms are underreported, thus explaining the lack of published cases and papers. The vast majority of the reports encountered in the literature are associated with the complications that develop from these aneurysms. The most common complications include deep venous thrombosis, thrombophlebitis, thrombosis of the aneurysm, recurrent pulmonary embolisms and death. Here we present the case of a patient with bilateral common femoral and popliteal vein aneurysms and pulmonary embolism that occurred in two separate occasions, the second one despite anticoagulation. We opted to treat the common femoral artery aneurysms first. We performed bilateral common femoral vein explorations with aneurysmectomy and primary venorrhaphy with excellent results.
