It Takes A Village - Improved Outcomes In Complex Wound Care Patients With Care Integration And Continuity

OBJECTIVES: Expansion of critical care services has enabled long term survival of patients with diseases and resulting increasingly complex wounds.
METHODS: The course of four patients with necrotizing fasciitis causing complex wounds was reviewed retrospectively.
RESULTS: All four patients had severe sepsis and multi system organ failure on presentation. Multiple surgical procedures from various specialties were required for all. At the conclusion of treatment all had returned to fully independent function. The integration and treatment planning of the treatment teams was reviewed.
CONCLUSIONS: Maximal patient outcomes are achieved when care is integrated with consistent caregivers. This is especially true when the same team treats patients for the entire treatment course from injury, acute care, long term care and rehabilitation.
