Risk Factors Combined Ehit And Recanalization After Endovenous Thermal Ablation

OBJECTIVES: Endovenous thermal ablation (EVTA) in the form of radiofrequency ablation (RFA) or endovenous laser ablation (EVLA) has become the primary treatment modality of chronic venous insufficiency. Rates of endovenous heat induced thrombosis (EHIT) complications and recanalization can be found in the literature. However, there is a paucity of literature assessing the risk factors of those who developed EHIT and recanalization. We assessed the incidence of those who developed both EHIT and recanalization and associated risk factors.
METHODS: A retrospective analysis of 13,623 procedures performed in 4,058 patients was performed. Data were collected from follow-up visits within 1 week of procedure, every 3 months for the first year, and every 6 months thereafter. Recanalization was defined as >500 ms for the GSV, SSV, and AASV and as >350 ms for the PV. Any patients who developed EHIT 1,2,3, or 4 were recorded. Data for EHIT and recanalization were also correlated with age, gender, laterality, presenting symptoms of CEAP, BMI, and treated targeted vein. Multivariate logistic regression was used.
RESULTS: From our cohort, univariate descriptors are mentioned in Table 1, showing the difference in patients’ characteristics between the EHIT recanalization group vs patients with no EHIT nor recanalization on follow-up. Multivariate logistic regression controlling for the covariates was performed. Recanalization is strongly associated with EHIT occurrence in our population (OR 10.7, p-value <0.001). Laser ablation was associated with less risk of EHIT (OR= 0.44, p-value <0.001).
CONCLUSIONS: From our data, EHIT showed association with increased recanalization on multivariate regression. Further analysis is needed to delineate other contributing factors.
