Background:This video shows an aortic endarterectomy performed on a 63 year old female with severe lifestyle limiting claudication who was found to have a "coral reef" aorta with peri-visceral aortic stenosis and infrarenal aortic occlusion. The patient was also noted to have occlusion of her right renal artery, and severe stenosis of her superior mesenteric and left renal arteries. Methods: Aortic endarterectomy was performed via a thoracoabdominal incision. Patient also required aorta bi-femoral bypass grafting due to severity of iliac arterial disease. Results: Patient underwent successful aortic endarterectomy with primary closure and subsequently end to side aorta bi-femoral bypass graft. Conclusions: Aortic endarterectomy remains a viable option for select patients.
